The School Clinic, directed by a Registered Nurse, provides for the health care needs of the school community. Our mission is to provide direct health care services to students and staff, and to promote health and wellness.


Services offered by the clinic include: 


  • immediate care for students who are ill or injured

  • medication administration

  • collaboration with students, parents, teachers, and other staff as needed

  • maintenance of student health records

  • documentation of State required health screenings

  • care for students with chronic health conditions


The role of the clinic staff is to report signs and symptoms of health problems and encourage individuals to contact their own health care provider for diagnosis and treatment.


Physical Exam Requirements

 

A School Entry Health Examination (Florida Form DH3040) documenting the results of a physical examination given within the last 12 months is required for all newly enrolled students. This exam must be completed by a qualified medical professional in the United States. 

 

Immunization Records

 

All new parents must provide the school with an Immunization record, documented in the English language. The record must have evidence of each vaccine required for your child's grade at school. The Florida Immunization Form DH 680 (form must be obtained from your FL physician) must be used to document receipt of immunizations required for entry and attendance in Florida schools. A request for a religious exemption from immunization requirements must be presented to the facility/school on the Department of Health's Religious Exemption Form Immunization (DH 681 Form). The Form is issued ONLY by county health departments and ONLY for a child who is not immunized because of his/her family's religious tenets or practices. The student will not be able to attend class without this. 

 

Day Students must get the Immunization History form from their Florida pediatricians office or health department.

Health care providers may grant a Temporary Medical Exemption for children who are in the process of completing any necessary immunizations. 

 

Magnus Health Portal

 

All parents are given access to our school web-based health computer system called MAGNUS.

The program enables parents to access their child's health information, update medical information, give permission to treat and communicate with the nurse.

Keeping Your Child Home


Please have consideration for other students as well as your child by keeping them at home if:

  • your child vomits or has diarrhea prior to coming to school, a period of 24 hrs without symptoms must pass before returning to school

  • your child has an elevated temperature of 100 or over, a period of 24 hrs without fever medication must pass before returning to school

  • your child has an itchy red eye with discharge, please visit the pediatrician prior to returning to school

  • Your child has a cough/cold symptoms, please monitor and consult with your division nurse; consider testing for COVID

MEDICATION GUIDELINES

Does your child need to take medication while at school or on a class trip? All prescriptions require parental permission and must have a medication authorization form completed in Magnus Health. We also require action plans for certain conditions.
